SERVICE COORDINATION DEPARTMENT

Service Coordinator - Bilingual Spanish/English

Location: Berks Office

Position Summary: The Service Coordinator (SC) is a position within the Home and Community- Based Services/Service Coordination department.  The SC position is funded through the Department of Human Services.  The Service Coordinator plays an important role in ensuring that the participants receive the services and support that meet their assed needs.  The Service Coordinator works closely with the and at the direction of the participant, whenever possible, to assess, identify their needs, plan and coordinate their services and monitor quality of services including MA State Plan Services and other medical, social, and educational services.  The Service Coordinator monitors these identified services in order to support the participant in their independent living environment.

Candidate must be experienced in multi tasking ,working well with the public, and posess the abilty to identify how they can direct a caller. Must have attention to detail capabilities and be driven to achieve results. HIPAA compliance is critical in this position. Proficiency in Microsoft Word, Excel, Powerpoint and basic office duties are required. Must be willing to drive to corporate office of training and meetings. Bilingual in Spanish a plus. Position is temp-to-regular based on performance.

NURSING HOME TRANSITION DEPARTMENT

Nursing Home Transition Coordinators - English-Speaking

Locations: Bethlehem area and Lancaster County

Position Summary: The Nursing Home Transition Coordinator (NHTC) is responsible for overseeing the transition of consumers moving from nursing homes to a residential setting. The NHT Coordinator will work collaboratively with numerous social service agencies, family members, nursing home staff, Ombudsmen and therapists to successfully plan and organize transition.  Candidates need a Bachelors' Degree in Social Work or a related field. Preferred candiate should have three years in community-based services. Ability to work independently and build relationships with Social Workers in Nursing Homes.  Must have good communication skills.

Employment Specialist - Bilingual Spanish/English

Location: Berks Office

The Ticket to Work Program Specialist is part of the Independent Living Department. The functions of the role include facilitating the Social Security Ticket to Work program.  Bachelor's Degree in Human Services, Human Resources or related field is required.  Minimum two years work experience with job seekers with disabilities or working at a Center for Independent Living. Experience working in a Ticket to Work program is preferred.

AIM is dedicated to advancing the rights of persons with disabilities in order to promote a full life in the community through the prevention, and elimination of physical, psychological, social and attitudinal barriers which serve to deny them the rights and privileges common to the general public.  Services are not limited by type of disability or age.

The Service Coordinator plays an important role in ensuring that participants receive the services and supports that meet their assessed needs.  The Service Coordinator works closely with and at the direction of participants whenever possible, to assess; identify needs; plan and coordinate services; and monitor quality of services.  The Service Coordinator monitors these identified services in order to support the participant in their independent living environment.

Ideal candidates will:

  • have a Bachelor’s Degree in social work or related social sciences;
  • reside in the region specific to the position;
  • preferably have at least three years of experience in community-based social work;
  • be able to work independently from remote sites with minimal supervision; and,
  • have a valid driver’s license and an insured vehicle.
